Steps to activate your MyChart account with an activation code
- Go to mychart.baptistonecare.org and choose "Use My Activation Code"
- Enter your 15 digit code, last 4 digits of social security number and date of birth
- Create a 6-20 character username, password and security question
- Enter your email address
- Accept user agreement
Steps to activate your MyChart account without an activation code
- Go to mychart.baptistonecare.org
- Click "Sign Up Now"
- Enter the requested information and click "Submit"
